SORB has a warm, welcoming and friendly environment. A personal agent (called a PA) will be someone who works with you to help you achieve your dreams and goals. Your PA will listen to you, help you solve problems, and provide you with information and resources. Your PA will also work with you to develop a plan to provide services for you that is centered on your needs, goals, and dreams. Your PA will respond to you as quickly as they can – whether you need a returned phone call, a referral to a resource, help solving a problem, or just someone to talk to.


The first thing that happens is a meeting will be scheduled with you, the people in your life who you want to attend and your Personal Agent (PA).
At the meeting, you will be provided with information and documents to sign and an Adult Needs Assessment will be completed.

Your PA will work with you and others at future meetings to get information about your hopes, dreams and goals. You will have the opportunity to decide who you would like to hire to help you with your support needs, what kind of work you are interested in, and talk about what you would like your life to look like.

A meeting will then be scheduled so you can look at the documents prepared based on what you said during previous meetings. One of those documents is an Individual Support Plan (ISP) that lists everything you will receive during the ISP year. If you approve of those documents, they will be signed and your plan will be put into place. Your plan lasts for one year and will be redone once a year after that.

Your PA will help you to interview people to hire if you don’t already have someone in mind.
